• All Paperwork needs to be completed by the athlete's parents for the upcoming school year. 

     

    Medical History & Physical- Only 2020 forms will be accepted

       

    Medication Permit- This form must be completed for athletes that plan on carrying any prescription drugs (i.e. epi pen, inhaler, etc.) on campus.  The prescribing physician must complete this form annually.  

    Places to get a Physical Done- We prefer athletes see their primary care physician to get their physical but if that option is not available here are a few other options.
